Cryosurgery for Lung Cancer

Lung cancer is the leading cause of cancer death in both men and women around the world. Lung cancer is caused by smoking and the use of tobacco products in about 90% of cases. Other elements that can lead to lung cancer include radon gas, asbestos, air pollution exposures, and chronic infections. Know more about the causes of lung cancer.

What is it?

Normal cells in the body die at a specific point in their life cycle, prohibiting an overabundance of cells. Cancer cells, on the other hand, continue to develop and reproduce as a result Tumors form. Lung cancer is a type of cancer that begins in the lungs and affects a person’s capacity to breathe.

It is generally categorised into NSCLC and SCLC stages using the TNM method, which stands for:

  • T—the main tumor’s size: Has it spread to other organs or tissues in the area?
  • N—nodes: Has the cancer spread to nearby lymph nodes?
  • M—metastasis: Has the cancer spread to other organs, such as the brain, bones, lungs, or other parts of the body?

The Importance of Genomic Alterations in Lung Cancer

The Atlas Research Network detected changes in kinase protein levels in 230 lung adenocarcinomas after molecular profiling. Fusion events were discovered in the ROS1, RET, PRKCB, NTRK, MET, and ALK genes in lung adenocarcinomas, and in the PRKCB, PRKCA, PKN1, FGR, FGFR1, FGFR2, and FGFR3 genes in lung squamous cell carcinomas.

Cryosurgery

Only one type of treatment, Cryosurgery, will be discussed in this article. Patients with lung cancer who are not hopefuls for lung resection due to the advanced stage of the disease, the patient’s poor general condition, or poor respiratory function, and who have tumour recurrence after radiotherapy, chemotherapy, or lung resection, as well as those with localised lung cancer who refuse to receive operative therapy.

Cryosurgery is a procedure in which tumours are frozen and then allowed to resorb in their natural environment (in situ).  Cryosurgery is an important alternative as it has the potential for long-term survival.

Depending on the location and size of the tumour, cryosurgery for lung cancer can be done using endobronchial, direct intrathoracic (at exploratory thoracotomy), or percutaneous approaches.

Endobronchial cryosurgery can be used to treat a variety of conditions:

  • Histologically confirmed carcinoma of the trachea and bronchi;
  • Cancer that is inoperable due to the tumor’s location, performance status, or poor respiratory function;
  • Intraluminal tumours; that develop within the lining of a blood vessel.
  • Tumor recurrence after radiation, chemotherapy, or lung resection.

Patients whose cancer was first thought to be operable but was shown to have unresectable tumours at thoracotomy can benefit from direct intrathoracic cryosurgery.

Percutaneous cryosurgery can be used for a variety of purposes.

  • Small and solitary lung cancer that cannot or will not be treated surgically due to the patient’s poor performance and respiratory function;
  • Cancer that has progressed to the point where it is regarded unresectable due to tumour size and location;
  • Selected cases of lung cancer with a central location.

Nevertheless, percutaneous cryoablation, a simple and less invasive procedure, has shown promising results in the treatment of advanced non-small-cell lung cancer.
